Release
General Release and Waiver of Liability and Hold Harmless Agreement Please read carefully. This is a Release of Legal Rights
In consideration of being allowed to participate in any way in The Slime Labs activities, I, the parent and/or legal guardian of the minor named below agree to assume all risk of any kind of injury or damage my child, I, and any member of my party may receive or sustain as a result of participation, including property loss, property damage, or personal injury.

Child, me and my party will obey The Slime Lab’s code of conduct, signs and rules at all times (displayed in the premises).

*Those whose conduct or influence creates safety or discipline problems will be asked to leave.
I understand and acknowledge that I am fully aware and assume the risks (included but not limited to the risk of poisoning, allergic reactions to ingredients and materials used at The Slime Lab, the risk of any type of bodily injury, property loss or damage) of said minor child’s participation, my party’s and my participation in recreational activities at The Slime Lab and by the use of the slimes prepared at the premises. I recognize my responsibility to ensure that said minor participates only in those activities for which he/she has the required skills, age, qualifications and physical conditioning. I understand that The Slime Lab shall have no responsibility to pay for medical treatment and related costs if myself or said minor is injured.

I KNOWINGLY AND FREELY ASSUME ALL RISKS, BOTH KNOWN AND UNKNOWN, AND HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS, FOR MYSELF, THE ABOVE NAMED MINOR AND OUR HEIRS, THE SLIME LAB ,THEIR TRUSTEES, OFFICERS, EMPLOYEES AND VOLUNTEERS WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, or loss or damage to person or property associated with the presence or participation of the minor in my charge, my party and myself, WETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fully extent permitted by law.
Photo Release
I hereby grant The Slime Lab permission to use myself and above named minor’s in photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of The Slime Lab and will not be returned.

I hereby irrevocably authorize The Slime Lab to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein above named minor or myself appear. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge The Slime Lab from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
